Good day.
Under Immunisations there is a field to state whether the Immunisations are up to date, with Yes/No as answer. Could we add another field for a "Plan/description" when the answer to the question is no?

So it should only give the option when the answer is no. What type of field should it be? A text field where you can write anything or a select field where the user can select an option? Please explain a bit more of what the new field will require.

Yes, only give the option when the answer is no. And yes, a text field where we can write the information in ourselves.

For example: Immunisations up to date? Answer: NO ; "Plan"field pops up: I write "catch up, give 14 month injection today." or "too old for M+R, continue as per schedule"

Does it make sense?

Yes, it does. Is there a specific size this text box should be. We can make it that it can takes paragraphs if ever needed or it can simply take one line. To me it sounds a larger text field would be more appropriate.
