Zoe Clifford cbae2f916b Remove use of non-standard `char_traits<unsigned char>` from Pl_Buffer 
`basic_string<unsigned char>` implies use of
`char_traits<unsigned char>`.

This char_traits specialization is not standard C++, and will be
removed from LibC++ as of LLVM 18. To ensure continued LibC++
compatibility it needs to be removed.

There are two possible replacements here: `std::string` (e.g.
`std::basic_string<char>`), or `std::vector<unsigned char>`.

I have opted for vector since this code is dealing with a binary
buffer; though probably either way is fine (why does C++ even have
strings anyway??).

Jay Berkenbilt 5525c93124 Use QPDF_DLL_CLASS with Pipeline and InputSource subclasses 
This enables RTTI so we can use dynamic_cast on them across the shared
object boundary.

Jay Berkenbilt 79f6b4823b Convert remaining public classes to use Members pattern 
Have classes contain only a single private member of type
PointerHolder<Members>. This makes it safe to change the structure of
the Members class without breaking binary compatibility. Many of the
classes already follow this pattern quite successfully. This brings in
the rest of the class that are part of the public API.

The Apache License version 2.0 is now the primary license for qpdf.
However, users may, at their option, continue to use Artistic version
2.0.

Jay Berkenbilt 5d4cad9c02 ABI change: fix use of off_t, size_t, and integer types 
Significantly improve the code's use of off_t for file offsets, size_t
for memory sizes, and integer types in cases where there has to be
compatibility with external interfaces.  Rework sections of the code
that would have prevented qpdf from working on files larger than 2 (or
maybe 4) GB in size.
